OASIS Registered Nurse (HomeCare)
Provide compassionate, skilled nursing services to patients in their homes. Conduct comprehensive OASIS assessments, create individualized care plans, and coordinate care with an interdisciplinary team to support patient-centered outcomes.
Key Responsibilities
- Perform OASIS assessments and ensure required documentation is accurate and timely
- Develop and update individualized care plans based on patient needs
- Manage medications and support safe administration
- Provide wound care and IV care
- Educate patients and families on care plans, medications, and health management
- Coordinate with clinical teams to ensure continuity of care
Qualifications & Skills
- Current RN licensure and OASIS experience/knowledge
- Strong clinical assessment, care planning, and documentation skills
- Competence in medication management, wound care, and IV care
- Excellent communication and patient/family education abilities
- Ability to work independently in the community while collaborating with a team
