Licensed Practical Nurse (On-site)
Provide compassionate, direct patient care as part of an interdisciplinary healthcare team. Monitor and assess patients, record and update vital signs, and administer medications in accordance with the LPN scope of practice. Support treatments and procedures, document all care in the Electronic Health Record (EHR), and communicate effectively with team members to ensure safe, coordinated care.
Educate patients and families on care plans, health maintenance, and treatment instructions, while maintaining a respectful, inclusive, and patient-centered approach.
- Deliver bedside nursing care and support clinical procedures
- Monitor vital signs and observe patient condition
- Administer medications per LPN scope
- Document accurately in the EHR

Qualifications/Skills
- Active Licensed Practical Nurse (LPN) licensure
- Strong clinical assessment, medication administration, and documentation skills
- Effective communication and teamwork
- Compassionate patient care and ability to educate families
