Licensed Practical Nurse (LPN)
Provide direct, compassionate, resident-centered nursing care in a long-term and post-acute setting. Administer medications, monitor residents’ conditions, and support individualized care plans through close collaboration with an interdisciplinary team. Maintain residents’ dignity, safety, and comfort while delivering high-quality clinical support.
Key Responsibilities
- Deliver direct nursing care and supportive oversight for residents
- Administer prescribed medications accurately and safely
- Monitor vital signs/condition changes and report concerns appropriately
- Collaborate with an interdisciplinary team to implement individualized care plans
- Promote dignity, safety, and comfort in everyday care
Qualifications & Skills
- Current LPN license
- Strong clinical judgment and attention to resident safety
- Effective communication and teamwork with interdisciplinary staff
- Compassionate, professional bedside manner and resident-focused care approach
