Licensed Vocational Nurse (LVN) — Home Health
Provide compassionate, high-acuity nursing care to patients in their homes, delivering individualized support based on each patient’s needs. Manage complex conditions with a patient-focused approach while maintaining clinical excellence and safety.
Key Duties
- Deliver in-home skilled nursing care and monitor patient condition
- Assist with care planning and implement physician orders
- Assess, document, and report changes in patient status
- Provide medication administration and treatment support as ordered
- Support patients and families with education and compassionate care
- Coordinate with the healthcare team to ensure continuity of care
Qualifications & Skills
- Active LVN license
- Experience with high-acuity or complex patient care
- Strong clinical assessment, documentation, and communication skills
- Ability to work independently and remain patient-focused
- Sound judgment and commitment to safety and best practices
