RN Case Coordinator responsible for coordinating patient care across the continuum. You will develop, implement, and monitor patient-centered care plans, ensuring effective and safe transitions between inpatient, outpatient, and community settings.
Key duties:
- Coordinate care with physicians, nurses, and interdisciplinary teams
- Facilitate safe discharge and transitions across care settings
- Optimize resource utilization and support appropriate level-of-care decisions
- Work to reduce readmissions and improve patient outcomes
- Provide patient and family education and advocacy
Qualifications & skills: Registered Nurse with strong clinical judgment, care coordination skills, and the ability to collaborate effectively in a multidisciplinary environment. Demonstrated commitment to compassionate, patient-centered care and continuous professional excellence.
