Provide compassionate, hands-on personal care to hospice patients in their homes and care facilities, ensuring dignity, comfort, and quality of life.
Key Duties
- Assist with bathing, dressing, grooming, toileting, and oral hygiene.
- Support meal preparation and basic nutrition needs.
- Help patients with mobility and safe transfer assistance.
- Provide respectful, patient-centered care in home and facility settings.
- Collaborate with nurses and the interdisciplinary care team to support overall care goals.
- Travel within the service area to visit assigned patients.
