Primary Care Provider (In-Office & Telehealth)
Provide comprehensive primary care for patients of all ages, managing acute, chronic, and preventive health needs. Deliver both in-office care and occasional telehealth services, assessing patients, developing care plans, and coordinating appropriate follow-up.
Collaborate with in-office and virtual care teams to ensure integrated, patient-centered care. Perform clinical procedures within your scope of training and expertise.
Key Responsibilities
- Evaluate and treat acute and chronic conditions across the lifespan
- Provide preventive care, health education, and wellness guidance
- Develop and manage patient care plans and follow-up
- Coordinate with in-person and virtual care teams
- Perform procedures within established clinical training and scope
Qualifications & Skills
- Strong primary care clinical knowledge and patient assessment skills
- Ability to manage a variety of medical concerns and preventive needs
- Effective communication and teamwork with integrated care partners
- Comfort providing care via telehealth when needed
- Competence performing in-scope procedures
