Key Responsibilities
- Deliver hands-on clinical care in collaboration with RNs and physicians.
- Administer medications and perform related patient care tasks.
- Document patient care accurately in electronic health record systems.
- Support both in-person and virtual visits.
- Assist patients in using digital tools for care and communication.
- Contribute to workflow improvements and continuous learning.
Qualifications & Skills
- Current LPN license (and ability to practice within scope).
- Strong clinical skills for medication administration and direct patient care.
- Proficiency with electronic health records and tech-enabled care processes.
- Ability to partner effectively with multidisciplinary care teams.
- Excellent communication skills and a patient/member-centered approach.
