Private Duty Nurse (LPN) provides compassionate one-on-one skilled nursing care in patients’ homes.
Key Responsibilities
- Perform patient assessments and ongoing monitoring
- Administer medications and implement individualized care plans
- Provide wound care and manage trach/vent and G-tube care
- Collaborate with patients, families, and interdisciplinary care teams
- Document care accurately and respond promptly to changes in condition
Qualifications & Skills
- Current LPN licensure
- Strong clinical skills in medication administration and complex home care
- Ability to deliver compassionate, patient-centered care
- Excellent communication and teamwork skills
- Detail-oriented documentation and sound clinical judgment
