Key Responsibilities
- Perform comprehensive wound evaluations and ongoing reassessments
- Develop, implement, and update individualized wound care plans
- Coordinate care with interdisciplinary teams to support optimal outcomes
- Educate patients and staff on wound care best practices
- Ensure accurate, timely documentation in the EMR
- Support quality initiatives, infection prevention, and pressure injury reduction
