Hospice RN Admissions Nurse
- Coordinate and complete hospice admissions for patients in their homes.
- Conduct comprehensive nursing assessments to determine hospice eligibility and needs.
- Educate patients and families about hospice services, address questions, and obtain consent.
- Develop and initiate patient care plans in collaboration with the interdisciplinary team.
- Partner with physicians, care team members, and families to support continuity of care.
- Ensure regulatory compliance throughout the admissions process.
- Maintain accurate, timely clinical documentation and support smooth transitions of care.
Qualifications & Skills
- Current RN licensure and strong clinical assessment skills.
- Excellent communication and patient/family education abilities.
