Role Summary: Build and strengthen referral relationships to increase census for skilled nursing and post-acute care communities in the Magnolia–The Woodlands market.
Key Responsibilities:
- Develop referral networks and grow patient census for skilled nursing and post-acute care services.
- Educate healthcare partners on clinical capabilities, services, and outcomes.
- Coordinate care-transition communications to support smooth patient handoffs.
- Manage and optimize referral pipelines to ensure consistent lead flow.
- Identify and pursue market growth opportunities within the assigned territory.
- Maintain compliance with HIPAA and all applicable company policies.
Qualifications & Skills: Strong relationship-building and communication skills; knowledge of post-acute/skilled nursing care referral processes; ability to educate partners on clinical outcomes; experience managing referral workflows; strong understanding of HIPAA compliance.
