Under the direction of the primary care team, the Primary Care Nurse collaborates with providers to deliver patient-centered, safe, efficient, and equitable care, focusing on quality outcomes for an attributed panel.

This role involves managing triage calls/messages, partnering with patients and clinical team members, and performing duties aligned with organizational standards and values.

Position Highlights:

• Location: Sunset Hills, MO
• Schedule: Monday – Friday, 0800-1630; no nights, weekends, or holidays
• Training: Minimum three months on-site before transitioning to a hybrid work model; requires monthly on-site meetings

Qualifications:

• LPN: Diploma in Practical Nursing, active LPN license, 2-3 years preferred
• RN: AAS in Nursing, BSN, or equivalent experience; active RN license, 2-3 years preferred

Preferred Skills:

• Proficiency with Epic EMR
• Strong triage and computer skills
• Excellent time management and adaptability
