Provides skilled nursing care in patients’ homes, including assessments, care planning, medication education, dressing changes, IV therapy, access site care, and general nursing tasks following policies.
Supports patient safety and well-being by assisting with needs such as lifeline, meals on wheels, and public program instructions. Ensures timely documentation using the Epic system.
This Per Diem role involves patient visits on an as-needed basis, aligned with departmental policies, embodying our mission and standards.
Position Details:

Supports the Saint Louis area and surrounding counties with a variety of schedules, including part-time and full-time options, with weekday hours and on-call weekends/holidays.

Key Responsibilities:
- Deliver skilled nursing care per physician directives
- Assess patient needs and develop individualized plans
- Monitor progress, document services, and modify care as necessary
- Educate patients and families on health management and recovery
- Collaborate with healthcare teams to ensure seamless care
Qualifications:
- Valid LPN or RN license in Missouri
- BLS certification (American Heart Association)
- Valid driver’s license and auto insurance
- Preferred: 1 year experience in home health or related fields
