Role: Hospice RN On Call Triage/Float
Responsibilities:
- Provide compassionate after-hours hospice care through phone triage and symptom management.
- Conduct occasional field visits to assess patient needs.
- Ensure timely and safe interventions to support patients remaining at home.
- Assess patient conditions, update care plans in the EMR, and coordinate with interdisciplinary team members.
- Guide families through end-of-life decisions with sensitivity and professionalism.
Qualifications & Skills:
- Registered Nurse (RN) license.
- Strong communication and assessment skills.
- Ability to collaborate within a multidisciplinary team.
- Empathy and compassion for hospice patients and their families.
