Role: Hospice RN On Call Triage/Float
Responsibilities: Provide after-hours phone triage, symptom assessment, and urgent in-home visits for hospice patients.
Ensure timely interventions and compassionate end-of-life care.
Collaborate with an interdisciplinary team to update care plans in the EMR.
Support families through education and emotional guidance.
Qualifications & Skills: Registered Nurse (RN) license.
Strong clinical skills in hospice and palliative care.
Excellent communication and emotional support abilities.
Ability to work independently and collaboratively.
Key Duties: Deliver patient-centered care, respond to urgent needs, and maintain accurate documentation.
