Role: Provide person-centered support to individuals in a residential setting.
Responsibilities include:
- Assisting with personal care and daily activities.
- Supporting transportation, meals, and medication management when assigned.
- Encouraging community participation.
- Monitoring and documenting health and progress.
- Protecting individuals’ rights and confidentiality.
- Reporting incidents and following service plans and safety procedures.
