Role: Hospice LPN Float providing practical nursing care in patients’ homes within the Mt. Vernon area.
Responsibilities:
- Monitor patient conditions and document care accurately.
- Administer medications and treatments as prescribed.
- Assist with symptom and pain management.
- Coordinate with hospice nurses and interdisciplinary team members.
- Communicate updates to patients, families, and team members.
- Support patients and caregivers in understanding care plans.
