Role: Registered Nurse (RN) for Adult Home Health – High Acuity services.
Key Responsibilities:
- Provide skilled nursing care to medically complex adult patients in their homes.
- Conduct assessments, administer medications, and perform wound and IV care.
- Monitor high-acuity conditions and develop/update individualized care plans.
- Coordinate with physicians and therapists to ensure comprehensive patient care.
- Educate patients and families to promote understanding and compliance with care plans.
Qualifications & Skills:
- Current RN license.
- Experience in home health or high-acuity patient care preferred.
- Strong clinical skills in assessment, medication management, and wound care.
- Excellent communication and patient education abilities.
- Ability to work independently and collaboratively within a team.
