Role: Physical Therapist II
Responsibilities:
- Deliver advanced, evidence-based rehabilitation services to diverse patient populations.
- Evaluate and treat patients, developing individualized care plans to enhance mobility, safety, and independence.
- Collaborate closely with physicians and nursing teams to ensure coordinated patient care.
- Document patient progress thoroughly in the Electronic Medical Record (EMR).
- Educate patients and families on treatment plans and recovery processes.
- Participate in quality improvement initiatives to enhance care delivery.
