Role: Registered Nurse for Adult Home Health – High Acuity services.
Key Responsibilities:
- Provide skilled nursing care to medically complex adult patients in their homes.
- Conduct assessments, administer medications, and perform wound and IV care.
- Monitor high-acuity conditions and develop/update personalized care plans.
- Coordinate with physicians and therapists to ensure comprehensive patient care.
- Educate patients and families on health management and treatment plans.
Qualifications & Skills:
- Licensed RN with relevant clinical experience.
- Strong assessment, clinical, and communication skills.
- Ability to manage high-acuity cases and develop care strategies.
- Team-oriented with a compassionate approach to patient care.
