Role Overview
The Wellness Nurse (LPN/LVN) supports resident health in senior living settings by conducting wellness assessments, monitoring health changes, and ensuring clinical compliance. They assist with medication management, perform MAR/TAR audits, and maintain accurate documentation. The role involves communicating with families, physicians, and care teams to coordinate care and address concerns.
Key responsibilities include monitoring incidents and falls, supporting infection control, and contributing to clinical quality initiatives. The nurse also trains and supports care team members to uphold high standards of resident care.
Qualifications & Skills
- Active LPN/LVN license
- At least 1 year of experience in senior living or a clinical setting
- Strong assessment, documentation, and communication skills
- Knowledge of medication administration and compliance
- Problem-solving abilities and attention to detail
