Home-Based Physical Therapist

Role Summary: The Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and interventions, adhering to professional standards and ethics. They contribute to program development, quality improvement, and interdisciplinary collaboration.


Key Responsibilities:

  • Conduct evaluations and deliver treatments such as muscle strengthening, gait training, and equipment management for patients of all ages.
  • Create and update personalized treatment plans with clear goals, monitoring patient progress.
  • Complete accurate clinical documentation in compliance with guidelines.
  • Educate patients and caregivers on activities and care between sessions.
  • Coordinate with healthcare providers, providing updates and adjusting care plans as needed.
  • Manage appointment scheduling efficiently, including travel logistics.
  • Maintain departmental policies, safety, and equipment, ensuring quality and safety standards.
  • Monitor insurance authorizations and communicate with supervisors regarding scheduling or coverage issues.
  • Perform additional duties as assigned.

Qualifications:

  • High School Diploma or GED; Doctor of Physical Therapy (DPT) certification required post-2015.
  • Licensed Physical Therapist and valid Texas Driver’s License.
  • Valid auto liability insurance and BLS certification (to be obtained within 90 days).
