Position Overview

The Wellness Nurse (LPN/LVN) plays a vital role in supporting resident health and ensuring clinical quality in a senior living environment.

Key Responsibilities:

• Conduct wellness assessments and monitor changes in resident health
• Support medication management, including MAR/TAR audits and compliance
• Communicate effectively with residents, families, physicians, and care teams
• Monitor incidents such as falls and ensure appropriate follow-up care
• Maintain accurate documentation to meet regulatory standards
• Support infection control measures and clinical quality initiatives
• Train and mentor care team members to uphold care standards

Qualifications & Skills:

• Active LPN/LVN license
• Minimum of one year experience in senior living or clinical setting
• Strong assessment, documentation, and medication administration skills
• Excellent communication and problem-solving abilities
