Social Worker – Live Well Homecare

Job Summary
Responsible for delivering complex, professional services to patients and families within an interdisciplinary team setting.

Key Duties

Provide patient-centered care, ensuring effective communication and support

Collaborate with healthcare team members to develop and implement treatment plans

Assess patient needs and coordinate services accordingly

Document care activities and patient progress accurately

Ensure compliance with regulatory and organizational standards

Qualifications

Educational Requirements:
- High School Diploma, GED, or equivalent
- Master’s degree in Social Work
Experience:
- Preferred: Two years in Social Work Services
- Two years of Counseling experience preferred
Certifications and Licensure:
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with authorization to practice in the relevant state
- Basic Life Support (BLS) Certification (must obtain within 90 days of start date)
- SAFE Approach Level III (must obtain within 90 days)

Qualifications

Education:

- Two years of Social Work Services experience (preferred)
- Two years of Counseling experience (preferred)
