Role Overview: Provides patient education, promotes understanding of conditions, and ensures ongoing continuity of care. Acts as a patient navigator, coordinating pre- and post-hospital care, facilitating patient entry into care programs, and serving as a liaison between patients and healthcare teams.
Key Responsibilities:
- Coordinate multidisciplinary clinics and care activities.
- Communicate with patients via mail, phone, and in person to support their care plans.
- Assist in diagnosis and treatment support, collaborating with healthcare providers.
- Guide patients through care processes, reinforce education, and connect to support services.
- Maintain follow-up systems to monitor post-care patients.
- Provide nursing support and assist physicians during procedures.
- Educate team members and ensure compliance with policies.
Qualifications & Skills:
- Graduate from a Specialty Training Program in Nursing (required).
- Bachelor’s Degree in Nursing preferred.
- At least 2 years of RN experience required.
- Strong patient communication, coordination, and clinical skills.
- Ability to work in a clinical environment and collaborate effectively.
