Role Overview: This position involves assessing, planning, and implementing rehabilitative physical therapy programs to improve patient mobility, reduce pain, increase strength, and prevent deformities. Services are delivered across the continuum in a cost-effective, quality-focused, and patient-centered manner, ensuring compliance with regulatory standards.
Key Responsibilities: Develop and revise treatment plans with patient and family input; provide patient education and discharge instructions; evaluate ongoing treatment needs; document assessments and care plans; collaborate with physicians; and serve as a resource for staff and families.
Qualifications & Skills: Bachelor’s, Master’s, or Doctorate in Physical Therapy; licensure as a Physical Therapist; BLS certification; effective communication; customer service orientation; team player; basic computer skills; and adherence to ethical standards and confidentiality.
