Role Overview: The Care Coordination Daytime Team Lead (RN) oversees and mentors a care coordination team, supporting patients during hospitalization and in ambulatory case management.
Key Responsibilities:
- Conduct comprehensive patient assessments, including physical, psychosocial, and socioeconomic factors, to determine service needs.
- Develop and implement discharge and transitional care plans in collaboration with interdisciplinary teams, ensuring timely resource allocation and patient education.
- Respond to referrals, perform financial assessments, and facilitate post-discharge follow-up to support patient outcomes.
- Ensure accurate documentation in electronic health records and monitor compliance with departmental standards.
- Lead, coach, and mentor team members, participate in hiring, training, and performance evaluations.
- Support process improvement initiatives and contribute to quality and safety activities.
- Coordinate coverage, assist with scheduling, and facilitate team communication.
Qualifications & Skills:
- Minimum of 3 years recent care coordination experience in a hospital setting, with exposure to diverse medical populations preferred.
- Strong interpersonal, verbal, and written communication skills, with confidence in high-stress conversations.
- Proficiency in team organization, coaching, and change management.
- Licenses/certifications include RN, BLS, and applicable state or social work credentials.
