Physical Therapist – Home Healthcare

Role Overview: Responsible for providing primary physical therapy care to home health patients across multiple counties, supervising therapy staff, and coordinating interdisciplinary services to ensure optimal patient outcomes.


  • Key Responsibilities: Conduct assessments, develop and revise individualized care plans, manage patient progress, and perform routine and complex therapy procedures in patients’ homes.
  • Supervise PTAs and Home Health Aides, ensuring quality care delivery throughout the episode of care.
  • Provide patient and caregiver education to promote independence and self-care.
  • Communicate effectively with physicians, team members, and patients, and document all care accurately following regulatory guidelines.
  • Participate in performance improvement initiatives, staff training, and meet productivity standards.

  • Qualifications & Skills:
    • Graduate of an accredited physical therapy program.
    • Valid Physical Therapist license, BLS certification, and Georgia Driver’s License with insurance.
    • Minimum of 1 year of full-time PT experience, preferably in home health.
    • Strong assessment, rehabilitation, communication, and organizational skills.
    • Proficiency in home health operations, regulatory compliance, and documentation, with OASIS knowledge preferred.
