Key responsibilities:
- Conduct assessments aligned with scope of practice and standards.
- Address physical, psychological, social, cultural, and spiritual needs of patients and families.
- Develop and coordinate care plans, including patient education and discharge planning.
- Provide direct care such as treatments, medications, and diagnostic procedures.
- Evaluate intervention outcomes and adjust care plans accordingly.
- Collaborate with interdisciplinary team members to meet patient needs.
