Role Overview: This position involves assessing, planning, and implementing rehabilitative programs to improve patient mobility, reduce pain, increase strength, and prevent deformities. Services are delivered across the continuum with a focus on quality, cost-efficiency, and regulatory compliance.
Key Responsibilities:
- Implement and adjust treatment plans in collaboration with patients and families, providing education, discharge instructions, and referrals as needed.
- Conduct patient assessments, establish and evaluate treatment programs, and develop realistic discharge plans.
- Maintain accurate documentation of evaluations, care plans, and treatment sessions according to departmental standards.
- Adhere to ethical practices, maintain confidentiality, and ensure punctuality and professionalism.
- Deliver excellent customer service by fostering a positive environment, applying communication principles, and promoting teamwork.
- Participate in ongoing professional development, mandatory education, and orientation activities.
- Comply with organizational policies, standards, and codes of conduct.
Qualifications & Skills:
- Minimum Education: Bachelor’s, Master’s, or Doctorate in Physical Therapy.
- Licenses & Certifications: Valid Physical Therapist license and Basic Life Support (BLS) certification.
- Skills: Strong communication, customer service orientation, teamwork, and basic computer proficiency.
