Position Overview: The Home Infusion Nurse coordinates and delivers infusion therapy in clinical and home settings, managing referrals, verifying patient information, and providing direct care, education, and case management.
Key Responsibilities: Develop personalized care plans, prepare and administer infusion medications, perform vascular access procedures, educate patients and caregivers, monitor therapy progress, and ensure effective communication with healthcare teams. Maintain accurate documentation, respond to patient concerns, participate in protocol development, and support 24/7 on-call coverage. Travel to patient homes and clinical sites, oversee licensed practical nurses, and contribute to quality improvement initiatives.
Qualifications & Skills: Minimum of three years RN experience, preferred home health and IV therapy expertise, and graduation from a specialty nursing training program. Strong clinical assessment, communication, documentation, and teamwork skills are essential.
