Role Overview: This position involves evaluating and treating patients primarily in Rehab and LTAC hospitals, documenting and charging in the EMR, educating patients, families, and staff, and communicating patient progress within the team. Responsibilities include performing initial and discharge assessments, developing and implementing treatment plans, setting functional goals, and participating as an active team member. The role may also involve mentoring interns and participating in skills assessments.
Qualifications & Skills:
- Bachelor’s Degree in Physical Therapy required; Doctoral Degree preferred
- GA Physical Therapy license or eligibility
- BLS certification from the American Heart Association
- Minimum 1 year experience in Rehab, SNF, LTACH, or Acute settings preferred
- Basic computer literacy
Key Duties:
- Conduct evaluations, set goals, and create treatment plans
- Provide timely and effective therapy
- Identify educational needs and offer PRN as needed
- Collaborate with interdisciplinary team members
- Serve as a mentor for interns and participate in skills assessments
