Role Overview: The Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and interventions, adhering to professional and ethical standards. They contribute to program development, quality improvement, and interdisciplinary problem-solving.
Key Responsibilities:
- Conduct evaluations and administer treatments as prescribed by physicians for patients across all ages.
- Deliver therapies such as muscle strengthening, stretching, gait training, ADL training, and management of orthotic/prosthetic devices.
- Perform initial and ongoing patient assessments, documenting progress accurately and timely.
- Develop and adjust personalized treatment plans with clear short- and long-term goals.
- Instruct patients and caregivers on activities and exercises, providing clear written and verbal guidance.
- Maintain communication with healthcare providers regarding patient progress and plan adjustments.
- Manage appointment scheduling and travel logistics efficiently.
- Transport and set up portable therapy equipment, ensuring safety and readiness.
- Adhere to department policies on safety, quality, and infection control, and participate in departmental development activities.
- Monitor patient authorizations, insurance coverage, and communicate concerns to supervisors.
- Perform additional duties as assigned to support department operations.
Qualifications & Skills:
- High School Diploma or GED required; Doctor of Physical Therapy (DPT) degree after 2015 is mandatory.
- Licensed Physical Therapist and valid Texas Driver’s License required.
- Certifications in Auto Liability Insurance and Basic Life Support (BLS) within 90 days of employment.
- Reliable transportation and ability to operate portable therapy equipment.
- Strong communication, organization, and time management skills.
