Role: Registered Nurse in the Transitional Care Unit, a fast-paced environment caring for patients with diverse and complex medical conditions.
Responsibilities:
- Provide direct patient care, including assessment, diagnosis, planning, implementation, and evaluation within nursing standards.
- Promote patient and family-centered care, educating on illness management, post-treatment care, and medication administration.
- Collaborate with an interdisciplinary team of physicians, therapists, case managers, and pharmacists to ensure optimal patient outcomes.
- Develop and document individualized plans of care, communicate across the continuum, and evaluate physical, psychosocial, and emotional needs.
- Participate in activities that ensure patient safety, quality, and regulatory compliance, and engage in ongoing professional development.
Qualifications & Skills:
- Required: Nursing Diploma/Associate’s Degree, RN license, no prior experience needed.
- Preferred: Bachelor’s Degree in Nursing and at least 2 years of experience.
- Strong critical thinking, assessment, and clinical decision-making skills.
- Ability to work collaboratively within a multidisciplinary team.
