Clinical Nurse – Transitional Care Unit

Role Overview:

This position is for a Registered Nurse working in a Transitional Care Unit within a Level I Trauma Center. It offers flexible, PRN shifts, with two twelve-hour shifts every six weeks, focusing on providing high-quality patient care in a fast-paced, multidisciplinary environment.

Key Responsibilities:

  • Deliver direct patient care including assessment, diagnosis, planning, implementation, and evaluation in line with nursing standards.
  • Educate patients and families on managing illnesses, post-treatment care, and medication administration.
  • Participate in ensuring patient safety, quality, and regulatory compliance, along with ongoing professional development.
  • Develop and execute personalized care plans collaboratively with the healthcare team, considering patient preferences and barriers.
  • Assess physical, psychosocial, and emotional needs, applying critical nursing skills to deliver holistic care.
  • Work in a safety-sensitive environment, demonstrating alertness and safe practices at all times.
  • Qualifications & Skills:

    • Minimum education: Nursing Diploma or Associate’s Degree in Nursing.
    • Licensure: RN license required.
    • Preferred: Bachelor’s Degree in Nursing and at least 2 years of experience.
