Role Overview: Provides patient care coordination, education, and ongoing support within a multidisciplinary care program. Acts as a liaison between patients and healthcare providers, facilitating entry into care programs and ensuring continuity of care.
Key Responsibilities:
- Coordinates pre- and post-hospital care and multidisciplinary clinics.
- Guides patients through the care process as a patient navigator.
- Provides education to promote understanding of conditions and ongoing care needs.
- Supports diagnosis and treatment by collaborating with healthcare teams.
- Communicates with patients via mail, phone, and in person, and coordinates activities across clinics and support services.
- Maintains systems for timely follow-up and surveillance of patients.
- Assists with nursing care, procedures, and educates team members.
Qualifications & Skills: Graduate from a specialty training program (required), Bachelor’s Degree in Nursing (preferred). Strong communication, coordination, and patient education skills are essential.
