Acute Care Clinical Pharmacist

Job Summary

The Pharmacist ensures the delivery of safe, effective, and cost-efficient medication therapy, developing and monitoring evidence-based care plans focused on patient outcomes and quality of life. They assess medications for indication, safety, effectiveness, and adherence, and monitor laboratory parameters and documentation.

Key Responsibilities

  • Provide pharmaceutical care tailored to the specific patient care area.
  • Coordinate resources for timely, efficient, and appropriate care.
  • Interpret, clarify, and correct physicians’ medication orders.
  • Review medication profiles to prevent drug interactions, contraindications, and sensitivities.
  • Evaluate prescribing, formulary, and therapeutic issues, suggesting alternatives as needed.
  • Consult with medical staff on drug therapy and monitor lab results.
  • Provide drug information to healthcare professionals and discharge medication counseling.
  • Participate in rounds with medical teams and calculate drug dosages.
  • Verify orders, dispense proper doses, and educate healthcare providers.
  • Prepare compounded and sterile medications using aseptic techniques.
  • Manage supplies, supervise pharmacy staff, and ensure compliance with standards.
  • Assist with departmental policy review and collaborate with healthcare teams to resolve issues.

Qualifications & Skills

  • Bachelor’s Degree in Pharmacy (required); Master’s degree preferred.
  • Strong knowledge of pharmacotherapy, laboratory monitoring, and medication safety.
  • Excellent communication, collaboration, and supervisory skills.
  • Ability to interpret complex medication orders and perform calculations.
  • Experience with compounding and sterile preparations is advantageous.
  • This role is primarily performed in a clinical environment, emphasizing patient-centered care, regulatory compliance, and teamwork to improve health outcomes.
