Role Overview: This position involves coordinating patient care, providing education, and facilitating entry into multidisciplinary care programs. The nurse acts as a liaison between patients and healthcare providers, ensuring continuity of care and supporting diagnosis and treatment.
Key Responsibilities:
- Facilitates patient onboarding into care programs and provides ongoing education about their conditions.
- Coordinates pre- and post-hospital care, multidisciplinary clinics, and patient activities.
- Serves as a patient navigator, guiding new patients and ensuring timely follow-up.
- Supports clinical activities, assists physicians, and educates team members.
- Communicates with patients via multiple channels and collaborates with healthcare providers to develop care plans.
- Coordinates support services, such as support groups and social services, to meet patient needs.
- Maintains systems for follow-up and provides nursing care as needed.
Qualifications & Skills: Graduate from a specialty training program (required); Bachelor’s Degree in Nursing (preferred). Must have clinical support skills, excellent communication, and collaboration abilities.
