Position Overview: The Home Infusion Nurse coordinates and delivers infusion therapy services across clinical settings, managing patient referrals, developing individualized care plans, and providing direct patient education and case management. This role involves administering medications, vascular access procedures, monitoring therapy progress, and collaborating with interdisciplinary teams to ensure safe, effective, and patient-centered care.
Key Responsibilities:
- Verify referrals, demographics, clinical history, and obtain necessary authorizations and consents.
- Assess patients thoroughly and develop tailored care plans, including venous evaluations.
- Administer infusion medications aseptically, perform vascular access, and conduct lab draws.
- Educate patients and caregivers on therapy protocols, equipment, and vascular care.
- Monitor treatment progress, adjust plans as needed, and communicate with healthcare providers.
- Ensure accurate documentation in electronic medical records and maintain compliance with standards.
- Respond to concerns, escalate issues appropriately, and participate in protocol development and quality initiatives.
- Provide clinical oversight to Licensed Practical Nurses and support 24/7 coverage through on-call duties.
- Travel to patient homes and clinical sites for therapy delivery and assessments.
Qualifications & Skills:
- Minimum three years of RN work experience required.
- Preferred experience in home health and IV therapy.
- Graduate from a Specialty Training Program in Nursing.
- Strong communication, clinical, and organizational skills.
- Ability to work independently and as part of a team.
