Registered Nurse Coordinator – Primary Care

Role Overview: This position involves serving as an expanded nursing role within an interdisciplinary team to promote preventative care, conduct screenings, coordinate disease management, and support primary clinical functions. The nurse assesses patient needs, develops individualized care plans, educates patients and families, and facilitates communication among healthcare providers. Key responsibilities include implementing clinical interventions, coordinating diagnostic tests, maintaining documentation in electronic health records, and participating in professional development.


Responsibilities: Assist in disease management and care coordination, ensure adherence to clinical guidelines, monitor patient progress, and support transitions of care. Administer medications and vaccines, manage IVs, and prepare laboratory specimens. Contribute to designing and evaluating care programs and maintain accurate documentation.


Qualifications & Skills: Requires a graduate from a nursing specialty training program, with a preference for a Bachelor’s Degree in Nursing. At least two years of RN experience, preferably in Med/Surg or hospital settings, is required. Strong clinical, communication, and collaboration skills are essential, along with the ability to educate and support patients and team members.
