Role Overview: A licensed physical therapist responsible for assessing, planning, and implementing rehabilitative programs to improve mobility, reduce pain, and prevent deformity in patients with disease or injury. Services are delivered across the continuum with a focus on quality, cost-effectiveness, and regulatory compliance.
Key Responsibilities:
- Patient Assessment: Collaborate with physicians to develop and evaluate treatment plans, goals, and discharge strategies, involving patients and families in goal setting.
- Treatment Planning and Care: Implement treatment plans, select appropriate activities, and modify goals based on patient progress. Provide education, discharge instructions, follow-up, and referrals.
- Documentation: Record evaluations, treatment sessions, family education, and discharge details according to protocols.
- Professional Conduct & Customer Service: Maintain ethical standards, confidentiality, punctuality, and effective communication. Foster teamwork, respect, and a positive environment. Apply service recovery principles and utilize AIDET practices.
- Professional Development: Engage in ongoing education, complete required competencies, participate in training, and adhere to licensure and hospital policies.
Qualifications & Skills:
- Master’s in Physical Therapy required; Doctorate preferred.
- PT license and BLS certification required upon hire.
- Minimum of 1 year experience in rehab, LTACH, acute care, or SNF settings.
- Effective communication, customer service orientation, teamwork, and basic computer skills.
