Role Overview:
Provides skilled nursing care in patients’ homes, evaluating their needs, developing personalized care plans, and coordinating services with the interprofessional team. Responsible for assessing patient preferences, cultural and emotional needs, and documenting care delivery to ensure quality outcomes. Promotes respectful, patient-centered interactions and practices evidence-based care. Evaluates patient condition changes, communicates updates to families and team, and updates electronic medical records accordingly.
Responsibilities:
- Assess patient needs, preferences, and barriers to involvement in care.
- Develop and implement individualized care plans with clear goals.
- Collaborate with patients, families, and care teams, ensuring effective communication across the continuum of care.
- Deliver culturally sensitive and empathetic care, advocating for patient and family-centered outcomes.
- Monitor patient status, inform caregivers of changes, and adjust plans as needed, documenting all activities accurately.
Qualifications & Skills:
- Associate’s Degree in Nursing or Nursing Diploma.
- Valid RN license and driver’s license.
- Minimum of 2 years of nursing experience; preferred Bachelor’s Degree and 2-5 years in home health.
- Strong critical thinking, communication, and collaborative skills.
- Ability to work safely and maintain alertness in a safety-sensitive role.
Key Duties:
- Perform home visits, provide skilled nursing interventions, and support patient self-care practices.
- Coordinate care with multidisciplinary teams and manage documentation.
- Ensure compliance with standards and promote quality patient outcomes.
