Role Overview: The Occupational Therapist provides age-appropriate rehabilitative care to patients in their homes, addressing physical, emotional, developmental, psychosocial, and educational needs. They collaborate with multidisciplinary teams to develop and revise individualized care plans, ensuring all patient goals are met.
Key Responsibilities:
- Assess and manage patients’ rehabilitative needs through routine procedures, therapeutic exercises, and patient education.
- Coordinate care with team members and communicate regularly with physicians regarding patient progress and changes.
- Develop, implement, and update personalized care plans based on patient status and physician directives.
- Educate patients and caregivers to support independence and recovery, utilizing verbal and written tools.
- Participate in staff training, continuing education, and quality improvement initiatives.
- Maintain accurate documentation within regulatory guidelines and communicate effectively with the care team.
Qualifications & Skills:
- Graduate of an accredited Occupational Therapy program.
- Valid Occupational Therapist license, BLS certification, and Georgia driver’s license with private vehicle insurance.
- Minimum of one year of full-time occupational therapy experience; home health experience preferred.
- Strong assessment, rehabilitative, organizational, and communication skills.
- Proficiency with computer systems, home health software, and regulatory compliance.
