Hospice Nurse – Live Well Homecare

Role Overview

This position involves providing specialized nursing care and support to patients in a hospice setting, focusing on pain and symptom management, emotional and spiritual support, and patient education for self-care. The nurse assesses patient needs, develops and implements individualized care plans in collaboration with a multidisciplinary team, and ensures effective communication across care teams. When serving as a case manager, the nurse coordinates all aspects of patient care, evaluates changes in patient condition, and updates care plans accordingly. The role emphasizes respect, cultural sensitivity, and patient-centered care, promoting dignity and comfort.

Responsibilities

Assess patient preferences, needs, and barriers to care, including cultural, emotional, and spiritual factors.

Develop, document, and communicate personalized care plans with clear goals, collaborating with patients, families, and team members.

Implement care with a focus on safety, evidence-based practices, and quality outcomes.

Evaluate patient condition changes, inform caregivers, and update care plans in the electronic medical record.

Promote respectful, equitable, and empathetic interactions with diverse populations, practicing collaborative problem-solving and advocacy.

Qualifications & Skills

Nursing Diploma or Associate’s Degree; RN license required.
Minimum of 2 years nursing experience; preferred experience in home health or hospice.
Valid Driver’s License.
Bachelor’s Degree in Nursing or related field is preferred.
