Hospice Nurse

Role Overview

This position involves providing specialized nursing care and support to patients in a hospice setting, focusing on pain and symptom management, emotional and spiritual support, and patient education for self-care. The nurse assesses patient needs, develops and implements individualized care plans in collaboration with a multidisciplinary team, and ensures effective communication across care teams. When serving as a case manager, the nurse coordinates all aspects of patient care, evaluates changes in patient condition, and updates care plans accordingly. The role emphasizes respect, cultural sensitivity, and patient-centered care, promoting dignity and comfort.

Responsibilities

  • Assess patient preferences, needs, and barriers to care, including cultural, emotional, and spiritual factors.
  • Develop, document, and communicate personalized care plans with clear goals, collaborating with patients, families, and team members.
  • Implement care with a focus on safety, evidence-based practices, and quality outcomes.
  • Evaluate patient condition changes, inform caregivers, and update care plans in the electronic medical record.
  • Promote respectful, equitable, and empathetic interactions with diverse populations, practicing collaborative problem-solving and advocacy.
  • Qualifications & Skills

    • Nursing Diploma or Associate’s Degree; RN license required.
    • Minimum of 2 years nursing experience; preferred experience in home health or hospice.
    • Valid Driver’s License.
    • Bachelor’s Degree in Nursing or related field is preferred.
