Role Overview
This position involves providing specialized nursing care and support to patients in a hospice setting, focusing on pain and symptom management, emotional and spiritual support, and patient education for self-care. The nurse assesses patient needs, develops and implements individualized care plans in collaboration with a multidisciplinary team, and ensures effective communication across care teams. When serving as a case manager, the nurse coordinates all aspects of patient care, evaluates changes in patient condition, and updates care plans accordingly. The role emphasizes respect, cultural sensitivity, and patient-centered care, promoting dignity and comfort.

Qualifications & Skills
- Nursing Diploma or Associate’s Degree; RN license required.
- Minimum of 2 years nursing experience; preferred experience in home health or hospice.
- Valid Driver’s License.
- Bachelor’s Degree in Nursing or related field is preferred.
