Role Overview: This position involves assessing, planning, implementing, and evaluating patient care to optimize health outcomes. The nurse manages utilization, applies evidence-based guidelines, and develops patient-centered treatment plans in collaboration with patients, families, and healthcare teams.
Key Responsibilities:
- Coordinate and monitor care, ensuring quality and efficiency.
- Develop systems for early intervention and ongoing assessment of health metrics.
- Design individualized care plans and foster teamwork for comprehensive care delivery.
- Evaluate diagnostic results, utilization patterns, and adjust treatment as needed.
- Maintain accurate documentation and serve as a resource for benefits and utilization management.
Qualifications & Skills:
- Minimum of two years RN experience, preferably in a hospital setting.
- Case management experience is a plus.
- Graduate from specialty nursing training; Bachelor’s Degree in Nursing preferred.
- Excellent collaboration, communication, and analytical skills.
