Registered Nurse Coordinator – Primary Care – Part Time

Role Overview: This position involves serving as an advanced practice nurse within an interdisciplinary team to promote preventive care, conduct screenings, and coordinate disease management services. The nurse provides primary clinical support, assists with patient triage, and develops individualized care plans based on assessments of healthcare, educational, and psychosocial needs.


Key Responsibilities:

  • Assess patient and family needs, involving them in treatment planning.
  • Facilitate communication among patients, providers, and healthcare team members to address barriers and interventions.
  • Educate patients and families on disease management, case programs, and clinical processes.
  • Implement clinical interventions following evidence-based guidelines and risk stratification.
  • Collaborate with primary care providers to ensure adherence to clinical guidelines and adjust therapies as needed.
  • Coordinate diagnostic tests, maintain documentation in electronic health records, and collect data for quality improvement.
  • Participate in ongoing professional development and incorporate current literature into practice.
  • Assist with discharge transitions, specimen collection, medication administration, and IV management within scope of practice.

Qualifications & Skills:

  • Minimum two years of RN experience, preferably in Med/Surg settings.
  • Graduate from a Specialty Nursing Training Program; Bachelor’s Degree preferred.
  • Strong clinical, communication, and organizational skills.
  • Ability to work collaboratively in a team environment and adapt to clinical needs.
