Role Overview: The Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and therapeutic interventions, adhering to professional standards and ethics. They contribute to program development, quality improvement, and interdisciplinary problem-solving.
Key Responsibilities:
- Conduct evaluations and deliver treatments such as muscle strengthening, gait training, ADL training, and device management for patients of all ages.
- Create and update personalized treatment plans with measurable goals, monitoring patient progress.
- Perform accurate and timely clinical documentation in compliance with guidelines.
- Educate patients and caregivers on activity routines and safety procedures.
- Coordinate with healthcare providers, providing updates and modifying care plans as needed.
- Manage scheduling efficiently, including travel logistics and equipment transportation.
- Ensure adherence to department policies, safety protocols, and quality initiatives.
- Monitor insurance coverage and patient authorizations, reporting concerns to management.
- Perform additional duties as assigned to support departmental goals.
Qualifications:
- High School Diploma or GED; Doctor of Physical Therapy (post-2015 required).
- Licensed Physical Therapist with valid licensure and credentials.
- Valid driver’s license and auto liability insurance.
- BLS certification within 90 days of employment.
