LPN Med Care Manager

Role Overview

The Licensed Practical Nurse (LPN) Medication Nurse provides licensed nursing support in senior care settings, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

Key Responsibilities:

  • Administer medications and treatments according to physician orders, adhering to the Six Rights of Medication Administration
  • Document accurately in MAR/TAR in real-time and transcribe physician orders to support care plan updates
  • Monitor residents for symptoms, adverse reactions, and physical, emotional, or cognitive changes
  • Maintain medication carts, complete narcotic counts, and ensure compliance with regulatory standards
  • Report refusals, reactions, and health changes promptly
  • Conduct Service & Health Updates (SHUs)

Qualifications & Skills:

  • Active LPN license in good standing
  • CPR and First Aid certification
  • Strong assessment, documentation, and observation skills
  • Preferred experience in senior care or medication administration

The role is vital in preventing medication errors, early detection of health changes, and ensuring resident safety through clinical precision and regulatory compliance.
