Role Overview: The OP Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and interventions, adhering to professional and ethical standards. They contribute to program development, quality improvement, and interdisciplinary problem-solving.
Key Responsibilities:
- Conduct evaluations and administer treatments as prescribed, catering to patients from adolescents to geriatrics.
- Deliver therapies such as muscle strengthening, gait training, ADL training, and orthotic/prosthetic management.
- Perform initial and periodic evaluations, documenting patient progress accurately and timely.
- Develop and adjust individualized treatment plans with clear goals.
- Educate patients and caregivers on activities and home exercises, providing clear instructions.
- Coordinate with physicians and healthcare team members, updating on patient progress and modifying plans as needed.
- Manage scheduling efficiently, including travel routes and equipment transportation.
- Maintain departmental policies, safety, and quality standards, participating in program development.
- Monitor insurance authorizations and communicate with supervisors regarding scheduling and coverage concerns.
- Perform additional duties as assigned.
Qualifications:
- High School Diploma or GED required; Doctor of Physical Therapy (DPT) degree needed, especially for post-2015 graduates.
- Licensed Physical Therapist and valid Texas Driver’s License required.
- Certifications include Auto Liability Insurance and Basic Life Support (BLS), with BLS required within 90 days of start.
- Reliable transportation and ability to carry portable therapy equipment are essential.
Skills & Experience: No prior experience required; candidates must hold necessary licensure and certifications.
